Terms and Conditions:

* One entry per person, not eligible on both dates. Only Individuals Over 18 Years of Age are Eligible. Must be Qualified for Bariatric Surgery. Winner will required to deposit $500 USD, for incidentals – which is refundable. Winner also agrees to perform and consents use of their before and after pictures, testimonials, video, images and anything else we deem acceptable. Winner also acknowledges that there are additional costs of $350 USD for BMIs over 49, $650 USD for Revisional Surgery and $350 for Hiatal Hernia Repair, $150 for Additional Companion, among other costs. Free Surgery is valued at $5,200 USD. Surgery must take place by May 31st, 2015. Patients must be present and are required to fill out a health questionnaire in order to be eligible.
